Memorial Hermann Heart & Vascular Institute Southwest opened in 2006 as Houston's first freestanding heart hospital. Located on the campus of Memorial Southwest Hospital, the Institute boasts innovative cardiovascular programs and leading-edge technology. The Institute features a patient-focused continuum of cardiac care provided by a dynamic team of affiliated cardiologists and cardiovascular surgeons who collaborate to create individualized treatment plans.

Cardiac Rehab
Whether you have had a heart attack, bypass surgery, angioplasty, heart transplant or valve replacement, or have angina, congestive heart failure or any other cardiac-related illness, you may benefit from the cardiac rehabilitation program at Memorial Hermann. Our goal is to help people with heart disease participate actively and confidently in their own recovery so they can return, as much as possible, to the life they enjoyed before heart disease.
The cardiac rehabilitation program is a three-phase, multidimensional, medically supervised program designed to help patients recover as quickly as possible from heart disease. With a comprehensive focus on exercise, education and lifestyle change, the program is designed to improve overall physical and mental functioning. Patients who participate in a cardiac rehabilitation program following heart surgery or a diagnosis of heart disease tend to have better outcomes in managing their condition.
*Physician referral is required for outpatient cardiac rehabilitation.

Pre-Registration for Inpatient/Outpatient Resources
Please fill out the pre-registration form and have your insurance information available. Once you have pre-registered using our online tool, you should also complete and sign consent forms in person in the Business Office.
When visiting the hospital for appointments, scheduled tests or procedure, please bring your insurance card and identification or you may fax a copy of the front and back to the Business Office at (713) 778-6214.

Parking Information
Surface parking is available in the Southwest lot located off of the Southwest Freeway frontage road. Enter the Heart & Vascular Institute through the main doors at 7787 Southwest Freeway.
